In the right hands, Photoshop's certainly capable of faking something like this. However, my guess is that it's real. I think the main reason people are calling fake on this is that the face is a slightly lighter shade than the body - but that's quite common to see in bodybuilding shows (I've never understood why people do it, but that's another story...). To take these photos, the kid's face would need to be photographed at the right angle, cut out, and blended into the main image. That's pretty tricky, and if it really is faked they've done it very well. So I'd be surprised if they then missed something as basic as matching the colours on face and body.
