What is the usual costs for getting a bloodworks done? Do you have to go through your doctor?

If your doc knows your AAS usage history he/she will probably run a limited panel on the "system" for Major Hormones,Lipids,Liver, Kidney & Prostate. Depending on your present cycle or previous cycle, you need to evaluate the results accordingly with the doc. I'm not sure what a private endo would charge for this or an expansion of the hormone panel.
